Documents Arrange as following sequence 

I. Application Form issued by Council Office.

2. 10 Rs. Notarized Affidavit. (Specimen Attached)

3. Three Specimen Signature attested by Principal of concerning center. (Specimen Attached) or by the gazetted officer attached

4. 10th & 12″ mark sheet and certificate

5. Marks sheet and certificate (degree/diploma) of concerning course.

6. Course Completion

7. working in Uttarakhand state in concerning post (i.e. Staff nurse, nursing officer, teaching position in nursing college etc.) of department of nursing only.

8. (Original registration of up, MP and Karnataka only 2 hard copy for other states)

9. aadhar card

10. Print out copy of registration fee. (Fee details on website)

Note: It is mandatory for the candidate to appear in person at the office with the above documents
