I. Application Form issued by Council Office.

2. 10 Rs. Notarized Affidavit. (Specimen Attached)

3. Three Specimen Signature attested by Principal of concerning center. (Specimen Attached)

4. 10th & 12″ mark sheet and certificate

5. Marks sheet and certificate (degree/diploma) of concerning course.

6. Course Completion

7. Working in uttarakhand state or domicile of uttarakhand state.

8. Original Registration of concerning state

9. aadhar card

10. Print out copy of registration fee. (Fee details on website)

Note: It is mandatory for the candidate to appear in person at the office with the above documents
