I. Application Form issued by Council Office.

2. 10 Rs. Notarized Affidavit. (Specimen Attached)

3. Three Specimen Signature attested by Principal of concerning center. (Specimen Attached)

4. Health visitor Marks sheet and certificate.

5. Course completion provided by principal of training center. 6. Working certificate.

7. Aadhar card.

8. Photocopy of ANM registration.

9. Registration fee Rs. 1239 through online mode by QR code.

Note: It is mandatory for the candidate to appear in person at the office with the ab0ve documents
